One study reported that 21% of <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3960290\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>children with food allergy experienced food insecurity<\/strong><\/a>. That means more than 1 million U.S. children live with both food allergy and food insecurity.<\/p>\n\n\n\n<p>Let\u2019s take a look at these two distinct but sometimes overlapping public health issues.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What is food insecurity?<\/h2>\n\n\n\n<p><a href=\"https:\/\/www.ers.usda.gov\/topics\/food-nutrition-assistance\/food-security-in-the-u-s\/measurement\/#:~:text=Food%20insecurity%20means%20that%20households,and%20other%20resources%20for%20food.\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Food insecurity<\/strong><\/a> is defined as not having enough food or the right kinds of food to eat. This is caused by not having enough money or not being able to get to a grocery store.<\/p>\n\n\n\n<p>People with food allergies, Celiac disease and other chronic diseases are at risk for food insecurity. This is because they may need special foods (dairy-free, gluten-free, etc.) for their diet. Sometimes it can be a challenge to find these special foods.<\/p>\n\n\n\n<p>There are two categories of food insecurity:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>low food security<\/li>\n\n\n\n<li>very low food security<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-embed is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe title=\"Ask the Allergist: What is food insecurity?\" width=\"720\" height=\"405\" src=\"https:\/\/www.youtube.com\/embed\/PCpIjZdP_L0?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\">What is low food security?<\/h3>\n\n\n\n<p>Low food security homes have enough food, but there is less variety. The food may not always be healthy.<\/p>\n\n\n\n<p>In 2020, 6.5% of <a href=\"https:\/\/www.ers.usda.gov\/webdocs\/publications\/102076\/err-298.pdf?v=4597.5\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>U.S. homes had low food security<\/strong><\/a>. One study found that in children with food allergies, 20.95% <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3960290\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>live with low food security<\/strong><\/a>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What is very low food security?<\/h3>\n\n\n\n<p>In very low food security homes, families make smaller meals or skip meals because there isn\u2019t enough food. They often have reduced food intake. It can be temporary or long term. Job loss and poverty are the most common reasons.<\/p>\n\n\n\n<p>Food insecurity can <a href=\"https:\/\/www.ers.usda.gov\/webdocs\/publications\/99289\/ap-084.pdf?v=9258.3\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>vary by ethnicity and race<\/strong><\/a>. According to the USDA:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>1 in 12 white, non-Hispanic people (8.1%) live in food insecure households.<\/li>\n\n\n\n<li>1 in 4 Native American individuals (23.5%) live in food insecure households.<\/li>\n\n\n\n<li>1 in 6 Latino individuals (15.8%) live in food insecure households.<\/li>\n\n\n\n<li>1 in 5 Black, non-Hispanic individuals (19.3%) live in food insecure households.<\/li>\n<\/ul>\n\n\n\n<p>During the COVID-19 pandemic, more people had problems finding enough food to eat. When people lose their jobs, it leads to more people not being able to buy the food they need for their home. The immune system sees the allergy-causing proteins as dangerous invaders. To fight it, the body creates an antibody called Immunoglobulin E (IgE). This triggers an allergic reaction. Food allergy symptoms can involve:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>hives on the skin<\/li>\n\n\n\n<li>your breathing<\/li>\n\n\n\n<li>your heart<\/li>\n\n\n\n<li>your digestive system<\/li>\n<\/ul>\n\n\n\n<p>A food allergy reaction can cause mild symptoms or severe symptoms. A severe allergic reaction to food can happen. This is called anaphylaxis and it is a medical emergency. People experiencing severe allergic reactions require emergency medication called <a href=\"\/anaphylaxis\/what-is-epinephrine\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>epinephrine<\/strong><\/a>. problems.<\/p>\n\n\n\n<p>Studies show that some racial and ethnic groups are more likely to have food allergies. <a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2720064\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Non-white\/non-Hispanic adults<\/strong><\/a> are more likely to have a food allergy. <a href=\"https:\/\/jamanetwork.com\/journals\/jamapediatrics\/article-abstract\/1738764\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Black and Hispanic\/Latino children<\/strong><\/a> are more likely to have a food allergy than white, non-Hispanic children.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What else can affect food insecurity and food allergies?<\/h2>\n\n\n\n<p>Issues regarding food insecurity and food allergy include:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Racial and ethnic disparities<\/h3>\n\n\n\n<p>Black and Hispanic\/Latino children are more likely to have both food allergies and food insecurity. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3960290\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Black children with food allergies<\/strong><\/a> are more likely to have low food security compared to white children.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Milk and egg allergy<\/h3>\n\n\n\n<p><strong>Peanuts, milk, and eggs<\/strong> are the most common food allergies in babies and young children. A study found that <a href=\"https:\/\/onlinelibrary-wiley-com.ezproxy.library.tufts.edu\/doi\/full\/10.1111\/pai.13019\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>children with both an egg and milk allergy<\/strong><\/a> were at increased risk for food insecurity.&nbsp;Parents of children who are allergic to eggs and milk have more problems finding safe foods for their children to eat.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Lower health literacy<\/h3>\n\n\n\n<p>Food insecurity is also associated with lower health literacy. Health literacy is an ability to understand basic health information and services. People with high health literacy are able to make more informed decisions about their healthcare.<\/p>\n\n\n\n<p>For people with food allergies, health literacy also involves reading food labels. It\u2019s important they be able to understand and identify allergens on food labels.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Challenges in managing allergen-restricted diets<\/h3>\n\n\n\n<p>Managing a food allergy and food insecurity is a challenge for many families. People with food allergies must avoid the foods that contain even small amounts of their allergens. Eating any amount of an allergen can result in a severe reaction with life-threatening symptoms. But avoiding food allergens is not always easy, especially for people on low incomes. With a limited budget and less access to grocery stores, allergen-free foods can be limited or not available at all.<\/p>\n\n\n\n<p>In addition, it can be expensive to manage food allergies. It can <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27244806\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>cost more to buy special foods<\/strong><\/a> for people with allergies. That can impact the family budget. The <a href=\"https:\/\/jamanetwork.com\/journals\/jamapediatrics\/article-abstract\/1738764\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>average cost for special foods<\/strong><\/a> is $285 per year per child with food allergies. Homes with children with food allergies spend $4,184 more per year per child. This includes doctor visits, paying out-of-pocket costs for epinephrine prescriptions, and purchasing special foods.<\/p>\n\n\n\n<p>Managing food allergies could drive families who may not have been food insecure to experience food insecurity. The systems that aim to support those with food insecurity are not quite as effective for people with food allergies.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Not enough screening for food insecurity<\/h3>\n\n\n\n<p>Approximately75% of doctorsdo not ask their patients if they can <a href=\"https:\/\/jamanetwork.com\/journals\/jamapediatrics\/article-abstract\/1738764\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>afford food and groceries<\/strong><\/a>. Most doctors don\u2019t know if their patients have money for food. Some don\u2019t know enough about food insecurity to talk to patients. They also don\u2019t know how to help their patients if they couldn\u2019t afford food.<\/p>\n\n\n\n<p>The American Academy of Allergy, Asthma &amp; Immunology (AAAAI) hosted a workgroup on screenings for food insecurity. The <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2213219821012484\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>workgroup created guidance<\/strong><\/a> on how to set up food insecurity screening. It includes federal resources and assistance programs. One consideration is to develop a script for doctors, physician assistants and nurses to follow when discussing food allergy with patients.<\/p>\n\n\n\n<p>A<a href=\"https:\/\/frac.org\/aaptoolkit\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong> food insecurity toolkit<\/strong><\/a> from the Food Research &amp; Action Center is available for doctors to use with patients. The toolkit is called <em>Screen and Intervene: A Toolkit for Pediatricians to Address Food Insecurity<\/em>. It teaches doctors how to screen patients for food insecurity. The toolkit also offers ideas on how to help them be able to buy food.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Food deserts in areas of poverty<\/h3>\n\n\n\n<p>Another problem for some families is living in a food desert. This means they live in an area that doesn\u2019t have a grocery store nearby with healthy or allergen-free food. <a href=\"https:\/\/www.ers.usda.gov\/webdocs\/publications\/82101\/eib-165.pdf?v=3395.3\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Food insecurity statistics<\/strong><\/a> show that approximately 19 million (6.2%) Americans live in a food desert.<\/p>\n\n\n\n<p>Food deserts can be found in urban and rural areas. Poverty is common in food deserts. To determine if an area is a food desert, researchers review:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>how many stores are in an area;<\/li>\n\n\n\n<li>how far it is to get to those stores;<\/li>\n\n\n\n<li>the cost of food;<\/li>\n\n\n\n<li>if people can afford it;<\/li>\n\n\n\n<li>how easy it is to catch a bus or subway to the store.<\/li>\n<\/ul>\n\n\n\n<p>Some people with limited access to supermarkets may have to depend on convenience stores and fast-food restaurants for meals. These meals may not contain the same quality nutrients as fresh food and may not be as healthy. But options may be limited for those dealing with both food allergies and food insecurity.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">The higher cost of allergen-free foods is not factored into SNAP benefits<\/h4>\n\n\n\n<p>The Supplemental Nutrition Assistance Program (SNAP) is the largest national program. It is commonly known as \u201cfood stamps.\u201d&nbsp;In 2019, 10.9% of <a href=\"https:\/\/www.ers.usda.gov\/data-products\/ag-and-food-statistics-charting-the-essentials\/food-security-and-nutrition-assistance\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Americans received help from SNAP<\/strong><\/a>.&nbsp; SNAP benefits allow people to buy foods of their choice at the grocery store. But the benefits might not stretch as far when allergen-free foods cost more. <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27244806\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><strong>Allergen-free foods<\/strong><\/a> are often 2-4 times more expensive.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Limited allergen-free substitutions in WIC food packages<\/h4>\n\n\n\n<p>WIC is the Special Supplemental Nutrition Program for Women, Infants, and Children. WIC offers food packages containing basics such as milk, eggs and bread. WIC also provides vouchers for fruits and vegetables. But there is no guarantee that allergen-free foods are available.
